All Pre-K through 12^th grade enrollment forms can be picked up at the school office.

Online documents will be available in August 2023.

Enrollment Information

All students entering grades Pre-K -12^th must have current immunization record on file to enroll.

Any child with a medical condition such as asthma, diabetes, food/environmental allergy requiring medication, etc will need to have a medical action plan signed by their doctor or have a school medical form completed and returned ASAP.

New students MUST bring the following items with them to enroll:

1.  Current shot record

2.  Birth Certificate

3.  Social Security Card

FOR HS ONLY: Current high school transcript. (for scheduling purposes)
